Fireball Crosswords is a 45-times-a-year pay-to-play crossword that comes by email in both Across Lite and pdf format.
To subscribe, you simply pay the subscription fee and indicate what email address you want the puzzles to come to.

The puzzles are hard. How hard? If you have to ask, too hard for you.

The 2013 subscription is for 45 puzzles. If you subscribe late, you will be sent the puzzles you missed.

A subscription is 45 puzzles and costs $18.90. You can also be a Cover-the-PayPal-Fee Tipper (which covers the fee that PayPal charges me, $19.77), Tipper ($24.50), Big Tipper ($30.00), or Medici ($60.00), or get a Lifetime Subscription ($500.00). A Lifetime Subscription gets you every Fireball puzzle there is until you die or Fireball Crosswords ends. So, for example, if Fireball goes daily, you get all the puzzles at no additional charge. In addition, you get to select an answer to appear in a future Fireball puzzle. (The answer has to be something that is acceptable for a standard crossword, so check with me before doing this option.)
